A father and daughter were killed in a workplace incident at Banagher Precast Concrete in Banagher, Co Offaly, on Monday 28 September. They were named as Mihai Chinez, 49, and Denisa Chinez, 24. Gardaí and emergency services were called at about 11.40am, and both were pronounced dead at the scene.

Both papers report that the pair were understood to have been preparing a concrete mould when it happened. They were originally from Romania and had lived and worked locally for many years. The Health and Safety Authority said it is aware of the fatal incident and has launched an investigation.

The company called the event a "devastating tragedy", said the circumstances are not yet fully known, and promised to cooperate fully with the investigation. Local councillor John Leahy described a "deep sense of shock" in the community and concern for colleagues who witnessed the incident. Fianna Fáil TD Tony McCormack offered condolences to the families and the community.

A funeral notice named the family, and a joint funeral Mass is planned for Thursday at St Rynagh's Church, Banagher. Postmortem examinations were due at Tullamore, and the cause of the incident has not been established.
